• WHAT TO DO IF INJURED ON THE JOB?

    • If you are injured on the job no matter how big or small you MUST report to your immediate supervisor and fill out the accident report.
    • If you do not need to seek medical treatment no further action is required by you.  The supervisor will report to the safety committee any accident that is filed.
    • If you seek medical treatment of any kind the following steps are required.
      • Inform your supervisor you are seeking medical attention right away.
      • Contcact ESD 112 at 1-800-749-5861 to:
        • Report the incident has occured
        • Provide details of the incident to the claims staff
        • Obtain a claim number
      • WHAT TO DO AT THE MEDICAL PROVIDER'S OFFICE
        • Inform the medical provider that this is a owrk related incident
        • Provide the medical provider with your claim number
        • Instruct the medical provider to forward all infomration to:
          • ESD 112 Workers Compensation Claims Department
          • 2500 NE 65th Ave, Vancouver, WA 98661-6812
          • Phone 360-750-7504; Fax to 360-750-9836
          • Complete the Physicians Initial Report (PIR - provided by the doctor)
          • DO NOT FILE AND L & I STATE FUND CLAIM FORM AT THE PROVIDER'S OFFICE!
          • Obtain a note with your work status from the attending physician
          • Schedule a return appointment
    • If medical treatment occurs during non-working hours, email your supervisor and the human resources office prior to the appointment if possible or immediatley following the appointment.
